Getting discharged? Do you need hospital aftercare? Our staff can assist in transitioning you back to your home and providing the necessary post-hospital care as needed. From helping you with your belongings to organizing transportation, helping with prescription medications, groceries, supplies and incontinence products, to coordinating your next appointment schedule. We are here to support you where and when you need it. Our staff possesses an uplifting nature with their ability to listen and understand your care needs more compassionately.
Care at Home
Do you need extra help at home? Do you need someone for a day or two? or do you need ongoing care while transitioning out of the hospital? Either way, we are here to support you where and when you need it. Our short hour visits can support your minimum-to-moderate care needs while the long hours can support moderate-to-high care needs 7 days a week. Each client is set up with the same staff for consistency (especially for dementia clients), with backup staffing support for zero missed visits and flexibility in cases where there are changes to your care plan and service schedule. We are here to complement the other hours you are receiving from the government.
Labs and Appointments
Your appointment companions, also known as ambulatory escorts, accompany you to your doctor’s appointments, social functions, and religious services, take you to shop, or simply stroll around the neighbourhood together. Our staff have a higher sense of responsibility and professionalism that you can depend on.
Shared Bookings
CareOnDemand provide shared booking arrangements (multiple individuals per visit) if you live in a home with other clients who also need care and require extra help. We take pride in the unique care we provide to our clients and the kind of partnership we give to our compassionate staff.
Developmental Care
Our specialized DSWs provide care to clients with developmental disabilities. Under a safe and comfortable space, they provide meaningful activities geared toward engaging the individual. Our staff provides the clients with a loving atmosphere of support, dignity and respect at all times.
Supply and Delivery of Incontinence Products
We carry a wide range of pads, protective underwear and briefs to help those who experience varying levels of incontinence. Not all incontinence products are created equal – so the product that is best for you is going to depend on your needs. We offer products of all absorption levels with leading brands as well as more economical choices.
Supply and Delivery of Grocery Items
We supply and deliver grocery products coming from Costco and Metro. Any items you need of the two will be delivered within 24-36 hours. Ideal for sourcing home supplies and everyday grocery needs.
